We accept referrals for all adults and children from four years of age.




Referral form


Children demonstrating any of the following will require a hearing test


  • Family history of hearing loss
  • Frequent history of ear infections
  • Frequently asking for repetitions
  • Risk factors such as developmental delay
  • Concerns regarding speech (eg: misarticulation)
  • Speaking loudly
  • Delayed speech and language development
  • Speech problems such as misarticulation
  • Regression in speech
  • History of infectious disease (eg: bacterial meningitis)
  • Trouble focusing or paying attention
  • Behavior difficulties
  • Learning difficulties
  • Poorly developed social skills
  • Poor comprehension of instructions
  • Having TV or phone volume louder
  • Complaint of ringing or buzzing sound in ears


    Adults experiencing any of the following will require hearing assessment


-         difficult picking out words in the presence of background noise

-         describes lacking clarity in understanding speech

-         complains of tinnitus/buzzing/ringing sounds in one or both ears

-         difficulty hearing on the phone or TV

-         speaks loudly and has trouble understanding or responding

-         depression, worry or anxiety related to the inability to hear

-         exposure to high noise levels on their jobs or during recreation

-         reports everybody mumbles

-         others say TV or radio is very loud

-         history of chronic diseases (eg: diabetes, heart disease, kidney disease); Patients with a history of depression, cardiovascular disease, diabetes, and dementia should have their hearing screened annually
